Root Cause Analysis (RCA) forms the cornerstone of troubleshooting in various industries, such as manufacturing, healthcare, software development, and banking. By identifying and addressing the fundamental causes of problems, RCA aims to prevent their recurrence and enhance processes. This method involves delving beyond surface symptoms to uncover the contributing factors behind an issue.

The essence of RCA is to pinpoint the root causes of problems, rather than merely addressing their symptoms. As a systematic approach to problem-solving, RCA focuses on understanding the reasons behind problems, instead of just treating their immediate effects.

This program is designed to be hands-on, featuring case examples and team exercises. Participants will enhance their skills in applying RCA tools and techniques, enabling them to implement continual improvements in their workplaces immediately.

By the end of the 2 days program, participants will be able to:

• Distinguish between systematic troubleshooting versus trial-and-error.

• Brainstorm and list potential factors
• The 5 Whys & other techniques
• Cause & Effect Methodology

Module 3: Taking Actions

• Dig deeper into the most probable cause(s)
• Conduct further investigations
• Based on identified cause, generate solutions
• Evaluate possible solutions for effectiveness

Activity 3 – Taking appropriate corrective actions

Module 4: A Avoiding Pitfalls

• Choose and implement the best solution(s)
• Mitigate the root cause from reoccurring
• Ensure implemented solutions are effective.
• Establish permanent solution

Activity 4 – Steps to ensure a working Permanent Solution

Managers, engineers, executives, supervisors or anyone who wish to introduce effective root cause analysis in their workplaces.

This is a ​2-days programme.

This course runs from 9:00am – 5:00pm with an hour for lunch and periodic short breaks.
 
This is an intensive session designed to include as much discussion as possible. Lunch and breaks provide an opportunity for further questioning and discussion with the instructors.
